Use of the website
This website provides general information about our clinic and a waitlist form for prospective patients. Information on this site is not a substitute for professional medical advice, diagnosis, or treatment. In an emergency, call 911.
Waitlist submissions
Submitting the waitlist form does not guarantee acceptance as a patient. We will contact you using the information you provide when registration opens.
SMS text messaging
If you opt in to SMS, you authorize Jefferson Medical Centre to send you transactional and healthcare-related text messages at the mobile number you provided. Message frequency may vary. Message and data rates may apply. Reply STOP to unsubscribe or HELP for assistance. Consent to receive SMS messages is not a condition of receiving medical care. Carriers are not liable for delayed or undelivered messages.
Age Requirements
Jefferson Medical Centre provides healthcare services to patients of all ages. Adults may consent to receive SMS messages for themselves. Parents or legal guardians may provide consent on behalf of minors where permitted by applicable law. Anyone opting into SMS communications confirms they have the authority to provide consent for the submitted phone number. Consent to receive SMS messages is optional and is not a condition of receiving medical care.
